Property Modification Requests

Property Modification Requests must be submitted to the ACC prior to beginning work on your home.  The ACC strives to respond to most requests within seven (7) days, but please note that this is a volunteer effort, and requests may sometimes be delayed due to committee members’ personal, business and travel schedules.  Please submit your requests as far in advance as possible!

Requests should be as detailed as possible:  plans, specifications, drawings, color swatches, pictures, full descriptions, and whether it is a change from what is currently there are all helpful pieces of information.

While all modifications must be approved in advance, the ACC has developed broad design standards for the community that you may use as a guideline when planning any changes.  Additional guidelines and restrictions may be reviewed by consulting sections 5 and 6 of our neighborhood covenants.

Request for Property Modification

  • Please indicate description of work to be done, including specific materials to be used, color(s) and any other pertinent information. For requests that include exterior paint, please indicate the manufacturer, color name, and color code of each paint to be used as well as its location (e.g. siding, trim, front door, etc.) Please also include the name and telephone number of the contractor(s).
  • Please indicate the projected start date of the project, estimated timeline, and expected completion date.

  • Please attach appropriate sketches, drawings and plans in the fields below. If you have more than five attachments, please send any additional information to acc@lostforrest.com.

  • By filling out this form, in accordance with the Declaration of Protective Covenants of Lost Forrest Homeowners’ Association, I am hereby applying for permission to make the indicated alterations to the premises. I attest that this modification will conform to the existing look and feel of Lost Forrest and will NOT depreciate the value of this community.
